MEDICAL QUESTIONS
Your dedicated staff is available during office hours to answer your questions regarding your medical condition and concerns. Please call our main phone number (719) 766-9000 (then press 1), and your call will be directed to the one of our staff members. If you have questions after hours that can't be handled during regular business hours, our answering service will contact Dr. Hanson if appropriate. If you have an emergency type situation, please call 911.

DISABILITY FORMS AND FMLA
All disability forms and FMLA paperwork is filled out by our practice coordinators. The fee is $35.00 per form and must be paid at the time the paperwork is left at our office. The turnaround time is typically seven days and at that time, your paperwork will be ready for you to pick up. Upon special request, we can mail the forms back to you or fax them to your employer.
